Here's what you can expect:
Initial Consultation: During your initial visit, we will complete your Diabetes Cycle of Care assessment.
Follow-up Appointment (3-6 months later): Return for a comprehensive review and additional assessments, which may include GP and Practice nurse assessment, ECG, GPMP/TCA evaluations, and referrals to Allied Health professionals and Endocrinologist as needed.
Our dedicated team will discuss the various options available to address your specific needs comprehensively.
DIABETES CYCLE OF CARE:
CLINICAL ASSESSMENT:
Weight, Height, and BMI Measurement: Every six months, twice during each cycle of care.
Blood Pressure Measurement: Every six months, twice during each cycle of care.
PATHOLOGY TESTS:
HbA1c Assessment: Once annually to evaluate diabetes control.
Total Cholesterol, Triglycerides, and HDL Cholesterol Check: Annually.
Microalbuminuria Test: Annually.
ALLIED HEALTH ASSESSMENT:
Comprehensive Eye Examination: Every two years by an Optometrist or Ophthalmologist.
Foot Examination: Every six months, twice during each cycle of care.
Iron Infusion Clinic
We are pleased to offer our specialised Iron Infusion Clinic at Eastgate Medical Centre.
It's important to note that, for your safety and well-being, all patients are required to undergo an initial consultation with one of our skilled doctors. During this consultation, the doctor will assess your specific needs and determine the suitability of Intravenous (IV) Iron Therapy for your condition.
Our dedicated team also accepts referrals from colleague GPs for the assessment and management of patients with iron deficiency who may benefit from iron infusion therapy.
